• Freedom of Information (FOI) requests

• Requests from police forces for evidence or intelligence

• Requests from health professionals (e.g. doctors, CPNs, social workers) for care plan information

• Referrals or enquiries from other statutory or partner agencies

To help us process your request efficiently, please ensure the following:

• Provide as much detail as possible about the purpose of your request.

• Upload any official documentation or authorisation, such as a signed GDPR request form, Data Protection Gateway request, or Section 29 / Section 115 form (if from a police force or other governing body).

• Ensure the request complies with data protection legislation (UK GDPR and Data Protection Act 2018).

Someone will then review all submissions and respond within statutory timeframes or as soon as practicably possible depending on the urgency and legal basis of the request.
